What is it

Rhinoplasty is a plastic surgery procedure aimed at altering the shape and/or function of the nose. It can be performed for aesthetic reasons, to improve the appearance of the nose, or for functional reasons, to correct breathing problems such as a deviated septum or nasal obstruction.

During rhinoplasty, the plastic surgeon may remove or add tissue to the nose, reshape the tip, reduce the size of the nostrils, or correct a deviated septum. The procedure is performed under general anaesthesia or conscious sedation and may be done using either an open or closed technique, depending on the surgeon’s approach and the complexity of the case.

Rhinoplasty is one of the most common plastic surgeries worldwide and can significantly improve both the nose’s appearance and the patient’s quality of life. However, like any surgery, it carries potential risks and complications, so it is important to choose an experienced and qualified plastic surgeon to perform the procedure.

Who is it for

Rhinoplasty is indicated for people who wish to improve the appearance of their nose or correct functional problems that affect breathing. It may benefit individuals with:

  • A nose that is too large or disproportionate to the face
  • A nasal tip that is too wide, upturned, or drooping
  • A deviated nasal bridge with depressions or humps
  • Nostrils that are too narrow or wide
  • Breathing problems caused by a deviated septum, nasal deformities, or other conditions

However, not everyone is a suitable candidate for rhinoplasty. The procedure involves risks and may not be appropriate for all cases. It is important to consult a qualified plastic surgeon to evaluate whether rhinoplasty is suitable for your specific situation and to discuss expectations and potential risks.

Scars

Rhinoplasty scars are usually minimal and hidden inside the nose or in discreet areas such as the base of the nostrils. The technique and type of incision vary depending on the patient’s needs and goals.

In open rhinoplasty, a small incision is made across the columella—the strip of tissue between the nostrils—allowing the surgeon to lift the skin and access the nasal structures more directly. Although this leaves a visible scar, it is typically small and well-concealed by the natural anatomy of the nose.

Healing is a gradual process that can take several weeks to stabilise and fade. Following the surgeon’s postoperative care instructions is essential for proper healing and minimal scarring.

Technical Description

Rhinoplasty involves reshaping the nose to improve its appearance and/or function. The steps of the procedure may vary depending on the patient’s individual needs and the surgeon’s technique, but generally include:

  • Anaesthesia: The surgery may be performed under local anaesthesia with sedation or general anaesthesia, depending on the case.
  • Incision: The surgeon makes incisions inside the nose (closed rhinoplasty) or across the columella and inside the nose (open rhinoplasty).
  • Remodelling: The surgeon reshapes the nasal structures by removing or adding cartilage, bone, and/or soft tissue to achieve the desired result.
  • Closure: The incisions are closed with fine sutures, and a nasal splint is applied to stabilise the nose.
  • Postoperative care: The patient is monitored and prescribed medication for pain relief and infection prevention. Instructions are given regarding head elevation and activity restrictions.
  • Suture and splint removal: Stitches are typically removed after about one week, and the nasal splint is usually removed after 7 to 10 days.
  • Follow-up: The patient attends follow-up appointments for recovery monitoring and evaluation of the final results.

Type of Anaesthesia

Rhinoplasty can be performed under:

  • Local anaesthesia with sedation
  • Regional anaesthesia
  • General anaesthesia

The choice depends on the extent of the procedure, patient and surgeon preference, and medical factors. The surgeon will discuss the best option with the patient before surgery.

Results and Benefits

Rhinoplasty results vary depending on the patient’s goals and the procedure’s complexity. Generally, the surgery aims to improve nasal appearance, correct congenital defects, fix septal deviation, or enhance breathing function.

Expected results include:

  • A more symmetrical and proportionate nose
  • A refined nasal tip and balanced facial appearance
  • Improved nasal airflow and breathing function

Final results take time to fully emerge, as recovery is gradual. It is important to maintain realistic expectations and discuss achievable outcomes with the surgeon before the procedure.

Recovery

Recovery varies by patient, but typically involves wearing nasal dressings and a splint for several days. Swelling and bruising around the nose and eyes are common and gradually subside during the first few weeks.

Patients should avoid strenuous activity and heavy exercise for several weeks to reduce swelling and minimise complications. Sleeping with the head elevated helps manage swelling.

Full recovery and final results may take up to a year as tissues settle and refine.

Post-operative Care

Following medical advice after rhinoplasty is crucial for safe healing and optimal results. Common recommendations include:

  • Rest: Rest in the first days after surgery, avoiding activities that increase blood pressure.
  • Dressings: Keep the nasal dressing in place as directed; it will be removed by the surgeon after a few days.
  • Hygiene: Clean the nasal area with saline solution to prevent infection, as advised by your doctor.
  • Diet: Soft or liquid foods may be easier to eat in the first days while nasal breathing is limited.
  • Sun exposure: Avoid direct sunlight and use high-factor sunscreen to prevent pigmentation changes.
  • Physical activity: Avoid exercise for several weeks to allow proper recovery.
  • Follow-up visits: Attend all scheduled appointments so the surgeon can monitor healing and make adjustments if needed.

By following these instructions carefully, patients can ensure a smooth recovery and achieve the best possible aesthetic and functional outcome.
